To make an application complete the following form(s) and submit by email to slm@doc.govt.nz.

For all applications

Complete the following form:

• Application form (PDF, 206K) or 
• Application form (Word, 80K)

If an application is outside the scope of work relating to these fees or where there is no fee indicated, you will be provided with a cost estimate for your approval prior to work commencing. Estimates are not binding and may change.

Public Works Act consents

For Administering Bodies or other Government Agencies (including LINZ Accredited Agents) seeking Ministerial consent under the Public Works Act 1981.

Complete the form below and include this with the above form as part of your application;

Reserves Act consents

For Administering Bodies or other Government Agencies (including LINZ Accredited Agents) seeking Ministerial consent under the Reserves Act 1977.
